Building a Reporting Culture: The Heart of Aviation Safety

Every Safety Management System depends on one thing: people telling you what is really happening. Hazard reports, near misses and honest questions are the early-warning system that lets you act before harm occurs.

But people only report when they trust what will happen next.

Why people stay silent

  • Fear of blame or discipline
  • A belief that nothing will change
  • Reporting that is slow, complicated or anonymous in name only
  • Leaders who react to the messenger instead of the message

What leaders can do

  • Respond quickly and visibly to every report, even if the answer is that no action is needed
  • Separate learning from discipline, with clear and fair criteria for the rare cases that justify action
  • Make reporting easy: short forms, mobile access, a named person to talk to
  • Share what was learned and what changed, so people see the value of speaking up

Measure the right thing

A rising number of reports is usually a sign of growing trust, not growing danger. Track how quickly reports are acknowledged, how many lead to actions and how many of those actions are closed.
